Designation of the Responsible Entity
The responsible entity for data processing on this website is:
Dr. Gunnar Frahn,
Treskowallee 92,
10318 Berlin
Info@praxis-checkpoint.de
The responsible entity decides alone or jointly with others on the purposes and means of processing personal data (e.g., names, contact details, etc.).
Revocation of Your Consent to Data Processing
Some data processing operations are only possible with your explicit consent. You can revoke your already given consent at any time. An informal notification by email is sufficient for the revocation. The legality of the data processing carried out until the revocation remains unaffected by the revocation.
Right to Complain to the Competent Supervisory Authority
As the data subject, you have the right to lodge a complaint with the competent supervisory authority in the event of a data protection violation. The competent supervisory authority for data protection matters is the data protection officer of the federal state in which our company is based. The following link provides a list of data protection officers and their contact details: https://www.bfdi.bund.de/DE/Infothek/Anschriften_Links/anschriften_links-node.html.
Right to Data Portability
You have the right to have data that we process automatically based on your consent or in fulfillment of a contract handed over to yourself or to third parties. The provision is made in a machine-readable format. If you request the direct transfer of the data to another responsible party, this will only be done if it is technically feasible.
Right to Information, Correction, Blocking, Deletion
Within the framework of applicable legal provisions, you have the right at any time to free information about your stored personal data, its origin and recipients, and the purpose of data processing, and, if applicable, a right to correction, blocking, or deletion of this data. For this purpose and for further questions about personal data, you can contact us at any time via the email address datenschutz@praxis-checkpoint.de or the contact options listed in the imprint.
SSL or TLS Encryption
For security reasons and to protect the transmission of confidential content that you send to us as the site operator, our website uses SSL or TLS encryption. This means that data you transmit via this website cannot be read by third parties. You can recognize an encrypted connection by the “https://” address line in your browser and the lock symbol in the browser line.
Server Log Files
The provider of the website automatically collects and stores information in server log files that your browser automatically transmits to us. These are:
- Browser type and browser version
- Operating system used
- Referrer URL
- Host name of the accessing computer
- Time of server request
- IP address
These data will not be merged with other data sources. The basis for data processing is Art. 6 para. 1 lit. b GDPR, which permits the processing of data to fulfill a contract or for pre-contractual measures.
Online Appointment Bookings – Doctolib
By using the online appointment function, your information will be transmitted to Doctolib and processed there. This website includes seals or widgets from Doctolib GmbH, Mehringdamm 51,
10961 Berlin. A widget is a small window displaying changing information. Our seal also works similarly, i.e., it does not always look the same but changes regularly. The corresponding content is displayed on our website but is retrieved from Doctolib servers at that moment. Only this way can the current content be shown, especially the current rating. For this, a data connection must be established from this website to Doctolib, and Doctolib receives certain technical data (date and time of the visit; the page from which the request is made; IP address used, browser type and version, device type, operating system, and similar technical information) required to deliver the content. This data is used solely for the provision of the content and is not stored or otherwise used. Our purpose and legitimate interest in this integration is to present current and correct content on our homepage. The legal basis is Art. 6 para. 1 GDPR and §25 para. 1 TDDDG. No storage of the mentioned data occurs on our part due to this integration. To make an appointment, you will be redirected to www.doctolib.de. Doctolib processes the data you provide there under its own responsibility. Further information on data processing by Doctolib can be found in the privacy policy of the site www.media.doctolib.com.
Doctolib provides authorities and other entities within the meaning of Art. 11 DDG with a central contact point through which they can communicate directly with Doctolib: contact-dsa-de@doctolib.com. Communication through this contact point can take place in English and German.
Contact Form
Data transmitted via the contact form, including your contact details, will be stored to process your request or to be available for follow-up questions. This data will not be passed on without your consent.
The processing of the data entered into the contact form takes place exclusively based on your consent (Art. 6 para. 1 lit. a GDPR). You can revoke your already given consent at any time. An informal email notification is sufficient for the revocation. The legality of the data processing carried out until the revocation remains unaffected by the revocation.
Data transmitted via the contact form will remain with us until you request us to delete it, revoke your consent to storage, or the purpose for data storage no longer applies. Mandatory legal provisions – in particular retention periods – remain unaffected.

Use of Google Maps
This website uses Google Maps API, a map service from Google Inc. (“Google”) to display an interactive map and create directions. Google Maps is operated by Google Inc., 1600 Amphitheatre Parkway, Mountain View, CA 94043, USA.
By using Google Maps, information about your use of this website (including your IP address) may be transmitted to and stored by Google on servers in the USA. Google may pass on the data obtained via Maps to third parties if this is required by law or if third parties process this data on behalf of Google.
Google will not associate your IP address with any other data held by Google. However, it is technically possible that Google could identify at least individual users based on the received data. It is possible that personal data and personality profiles of users of the website could be processed by Google for other purposes over which we have no influence. You have the option to deactivate the Google Maps service and thus prevent data transfer to Google by deactivating JavaScript in your browser. However, we point out that in this case, you will not be able to use the map display on our pages.
The Google privacy policy & additional terms of use for Google Maps can be found at https://www.google.com/intl/de_de/help/terms_maps.html.
Use of Google Web Fonts
This site uses web fonts provided by Google for the uniform display of fonts. When you call up a page, your browser loads the required web fonts into your browser cache to display texts and fonts correctly.
To do this, the browser you are using must connect to Google’s servers. This allows Google to know that your IP address has accessed this website. The use of Google Web Fonts is based on Art. 6 para. 1 lit. f GDPR. The website operator has a legitimate interest in the uniform presentation of the typeface on its website. If consent has been requested, the processing is based exclusively on Art. 6(1)(a) of the GDPR and § 25(1) of the TDDDG, insofar as the consent includes the storage of cookies or access to information on the user’s device (e.g. device fingerprinting) as defined by the TDDDG. Consent can be revoked at any time.
If your browser does not support web fonts, a standard font from your computer will be used.
More information about Google Web Fonts can be found at https://developers.google.com/fonts/faq and in Google’s privacy policy: https://policies.google.com/privacy?hl=de.
